Fees & Insurance
Private Pay – You pay me directly at the time of service through check or cash. Your visits are not restricted in any way, and can be 60 or 90 minutes in length at the rate of $140 per hour for individual, and $150 for couples, (pro-rated by the quarter hour).
Employee Assistance Program – This is a “no cost to you” option if your employer has this benefit available. (Check with your HR Dept.). It is a program limited to short- term solution focused counseling, assessment and referral if needed. I am a credentialed and contracted provider with a wide network of EAP’s nationwide.
Health Insurance– I am a credentialed and contracted “in-network” provider for most of the major insurance companies, such as Blue Cross, Blue Shield, United Healthcare, Coventry, Midlands Choice, Cigna, Value Options, etc. Each client’s plan is different so I have you confirm with your insurance company prior to our meeting, what your benefits and financial responsibility would be. I can work with clients on an “out of network” basis, and provide you with a billing statement to present to your insurance company for any reimbursement. In all cases, fees are due and collected at the time of the visit. Insurance visits reimburse for a 45-50 minute session.
Questions to ask your insurance company:
- Do I have mental health benefits, and if so what are they?
- What is my annual deductible, and has it been met?
- How many sessions are allowed per year?
- Do I need a referral from my Doctor?
- What is my co-payment or co-insurance per visit?
My standard fee is $140 per hour for individual, and $150 per hour for couples.
Email and telephone consultations are billed at the rate of $140 per hour (pro-rated per quarter hour).
*This includes calls or emails with client attorneys, doctors and any other party on behalf of the client.
To prepare any written report for attorneys, doctors or for insurance companies for purposes such as disability or leave of absence, etc., there will be a charge of $140 per hour, pro-rated by the quarter hour.
Full payment is due each session. If you have a co-payment, that is due as well. Please have your payment ready at the beginning of each session.
There will be a $25 fee on any returned check.
Missed or cancelled appointments
Your appointment is reserved for you. Since I am unable to fill a cancelled or missed appointment on short notice, I require notification of at least 24 hours in advance. If there is less than the 24 hour notice, I will need to bill you for the time. Insurance does not pay for missed appointments.
